Session Description: The UC Davis Health System created a Tethered Meta Registry (TMR) that integrates data from multiple EMR powered paper-less care process with administrative, laboratory, biorepository, imaging, and other niche clinical systems. The TMR consolidates over 2.1 million patient records and aligning them with 1 billion + clinical and administrative events and observations. Through the use of the TMR and EHR-enabled physician order set adherence, UC Davis significantly reduced mortality, hospital acquired infections, and readmissions. Most notably, the UC Davis Stop Sepsis in 60 Minutes Initiative has saved over 200 lives in the last three years.

Session Learning Objectives:

• Explain the UCDHS approach to leveraging the EHR to improve quality.

• Identify how UCDHS connects people, process, and technology to save lives and improve outcomes.

• Define the barriers to improving Sepsis mortality rates and describe the steps required to develop an EMR-based intervention for Sepsis Quality Improvement
